Observation

The base of a portable basketball toy set was broken. Cube organizer shelves in various classrooms appeared to post the risk of falling or tipping. Certain cube organizer and book shelves appeared to be in poor condition with flaking paint, bare wood and sticker residues. Forensic lights in the washroom area did not have tube guards. Outlets near sink in the Bird's room were not Tamper-resistant nor equipped with a ground fault circuit interrupter.
